Applying for aftercare after the end of full-time care for foster children
If a care relationship ends, aftercare or transitional care can be provided for young adults.
Agreements on this are made in the assistance planning.
When a foster child leaves a foster care relationship, this is referred to as a "leaving care process". It is accompanied by a specialist from the foster care service.
The aim of aftercare is, for example, to help the foster child become independent once they reach the age of majority.
The specialists from the Foster Children's Service accompany the foster children through reflective follow-up discussions, among other things.
The counseling should take place within a reasonable period of time after the end of the assistance in a form that is understandable, comprehensible and perceptible to the young adult.
