The Act on the Modernization of Statutory Health Insurance (GKV Modernization Act - GMG) in principle put all non-insured social assistance recipients on an equal footing with those insured by law with effect from 1 January 2004 and are now treated as "cash patients". All recipients of social assistance are now used for co-payments within the limits of the burden.

The other non-insured (short-term) welfare recipients can continue to receive health assistance.
